What is Spravato?
Spravato® (esketamine) is a prescription nasal spray for adults with depression. It works on the brain's glutamate system, which is different from how most antidepressant pills work. Many antidepressants take weeks to help, and Spravato may work sooner for some patients, though results vary.

Who is Spravato for?
Spravato is FDA-approved for adults with treatment-resistant depression, either on its own or together with an oral antidepressant. Together with an oral antidepressant, it is also approved for adults with major depression who have acute suicidal thoughts or behavior.
Dr. Sreepathi may recommend Spravato if you:
- Have tried at least two antidepressant medications without enough relief
- Still have symptoms that get in the way of daily life
- Need a faster-acting option because of how severe your depression is
What to expect during a visit?
Spravato is taken only in our office, because it is available through a restricted safety program (REMS). You use the nasal spray yourself while our team supervises. Afterward we monitor you for at least two hours, and we check your blood pressure before and after the dose.
Because Spravato can temporarily affect coordination and alertness, you will need someone to drive you home. You should not drive or operate machinery until the next day, after a good night's sleep. Please don't eat for two hours before your visit or drink liquids for 30 minutes before.
How often will I need treatment?
Most patients start with two sessions a week for the first four weeks. After that, treatment is once a week for four weeks, and then once a week or every other week, depending on how you're doing.
What are the possible side effects?
The most common side effects are feeling disconnected or dreamy, dizziness, nausea, sleepiness, spinning sensations, and a temporary rise in blood pressure. They usually go away within a few hours of the dose. Spravato isn't safe for everyone, so we review your medical history before you start.
Insurance
We accept most major insurance plans, including Medicare and Medi-Cal, and we check your benefits before you start.
